The cisterna magna is one of three principal openings in the subarachnoid space between the arachnoid and pia mater layers of the meninges surrounding the brain. The openings are collectively referred to as cisterns. The cisterna magna is located between the cerebellum and the dorsal surface of the medulla oblongata. Cerebrospinal fluid produced in the fourth ventricle drains into the cisterna magna via the lateral apertures and median aperture. The two other principal cisterns are the pontine cistern located between the pons and the medulla, and the interpeduncular cistern located between the cerebral peduncles. Cerebral spinal fluid can also be obtained by puncture of the cisterna magna.

In neuroanatomy, habenula originally denoted the stalk of the pineal gland, but gradually came to refer to a neighboring group of nerve cells with which the pineal gland was believed to be associated, the habenular nucleus. The habenular nucleus is a set of well conserved structures in all vertebrate animals. Currently, the Terminologia Anatomica term refers exclusively to this separate cell mass in the caudal and dorsal aspect of the dorsal thalamus, embedded in the posterior end of the medullary stria from which it receives most of its afferent fibers. By way of the fasciculus retroflexus it projects to the interpeduncular nucleus and other paramedian cell groups of the midbrain tegmentum. The habenula receives input from the brain via the stria medullaris thalami and outputs to many midbrain areas involved in releasing neuromodulators, such as dopamine, norepinephrine, and serotonin.

It is formed at the anterior perforated substance by the union of ⁕ a small anterior cerebral vein which accompanies the anterior cerebral artery and supplies the medial surface of the frontal lobe by the fronto-basal vein. ⁕ the deep middle cerebral vein, which receives tributaries from the insula and neighboring gyri, and runs in the lower part of the lateral cerebral fissure, and ⁕ the inferior striate veins, which leave the corpus striatum through the anterior perforated substance. The basal vein passes backward around the cerebral peduncle, and ends in the internal cerebral vein; it receives tributaries from the interpeduncular fossa, the inferior horn of the lateral ventricle, the hippocampal gyrus, and the mid-brain.

Nociceptin or orphanin FQ, a 17 amino acid neuropeptide, is the endogenous ligand for the nociceptin receptor. It is derived from the prepronociceptin protein, as are a further 2 peptides, nocistatin & NocII. The gene coding for prepronociceptin is located on Ch8p21 in humans. Nociceptin is an opioid-related peptide, but it does not act at the classic opioid receptors and its actions are not antagonized by the opioid antagonist naloxone. Nociceptin is a potent anti-analgesic. Nociceptin is widely distributed in the CNS; it is found in many regions of the hypothalamus, brainstem, and forebrain, as well as in the ventral horn and dorsal horn of the spinal cord. Nociceptin acts at the Nociceptin receptor, formerly known as ORL-1. The receptor is also widely distributed in the brain, including in the cortex, anterior olfactory nucleus, lateral septum, hypothalamus, hippocampus, amygdala, central gray, pontine nuclei, interpeduncular nucleus, substantia nigra, raphe complex, locus coeruleus, and spinal cord. The N/OFQ-NOP system is found in central and peripheral nervous tissue, where it is well placed to modulate nociception. Indeed, there is some evidence that nociceptin may be involved in the phenomenon of opioid-induced hyperalgesia.
